A recent study published in JAMA Psychiatry has drawn attention to a possible connection between attention deficit hyperactivity disorder and cognitive decline later in life. Media reports quickly summarized the findings with a striking number: a fourfold increase in dementia risk for people with ADHD. That figure sounds alarming, but it deserves closer examination before anyone accepts it as a straightforward cause and effect relationship.
The study itself is observational. It followed a large group of individuals over time and noted statistical associations between an ADHD diagnosis and subsequent diagnoses of cognitive impairment. What observational research cannot do is prove that ADHD directly causes dementia. A psychiatrist familiar with the data suggests the hazard most likely originates elsewhere. Several factors could explain the link without ADHD being the root cause.
For instance, people with ADHD often struggle with sleep, which is a known contributor to cognitive decline. They may also have higher rates of cardiovascular issues, substance use, or social isolation. Each of these can independently raise dementia risk. some symptoms of ADHD, like difficulty focusing or remembering tasks, overlap with early signs of cognitive impairment, which can lead to misdiagnosis or diagnostic delay.
The takeaway is not that ADHD guarantees dementia. Rather, the connection may reflect a web of lifestyle, health, and diagnostic factors that travel alongside ADHD. Understanding these pathways is more useful than fixating on a single scary number.
